Automobile Related Injury Process

Provided patient/family verbal information about Michigan No Fault Insurance law.

Educated patient/family about no fault insurance; role of case manager if appropriate given injury

Requested family report accident to agent.

Notified admitting and Practice Management about presence (absence) of auto insurance.

It appears the patient does not have auto insurance coverage due to driving uninsured (or other reason) and has no health insurance. A medicaid application will be started.

It appears that the person has auto insurance coverage but not other insurance coverage for other health care issues. Information about other options (DHS, County Health Plan) discussed with patient.

Information given to patient/family about insurance auto case manager contact and role.

Gave family copy of No Fault Booklet(s).

Patient was injured in the automobile (crash or related incident) while employed (and driving a company vehicle). This is likely to be a workman's compensation case.

Patient was reportedly the driver and noted to have a positive drug screen upon admission.

Patient provided information on Michigan's Drunk Driving Laws and provided the brochure "Cutting Drunk Driving". Patient offerred referral information for substance abuse treatment.

Discussed with patient his use of substances while driving and reviewed risks and dangers

Provided a brief substance abuse intervention. Patient is in the (pre-contemplative stage, contemplative, relapse stage).

Patient is/is not agreeable to seeking help at this time. Patient provided referral information and given (Insert specific agency or program or instructions).

If patient was injured as result of a known or suspected drunk driver: Provided patient and/or family information brochure and application for Crime Victim's Fund.

Reviewed with patient/family need for handicap parking permit. Provided them with application form. Assisted them with obtaining a completed application form.

Reviewed with them the need for handicap transportation. Reviewed availability of public tranportation resources in their area. Provided them with an application for public handicap transportation and information on how to complete the application process.

Reviewed with patient/family the implications of a suspected seizure (syncompe spell) for driving and the possible recommendation that they not drive for 6 months or until released to do so by a physician.

Patient's family raised concern about pre-existing physical and cognitive conditions and their ability to drive. Provided information to the family about the process to report these concerns to the Secretary of State. Agreed to ask the medical team to discuss these issues with the patient.

Discussed with patient the implications of this injury upon their driving habits. Asked him/her about his concerns for his driving safety. Suggested he/she consider the program Driving Safety for Seniors and provided him with an information brochure and contact information for local class.
