Washtenaw County Health Plan

Provided patient/family verbal information about the Washtenaw County Health Plan including eligibility requirements, what it covers, and application information.

In addition I assisted them with the application process. Required application information was faxed (Date).

Written information provided included: a copy (in English or language of the information brochure about WHP from their website (in French, Spanish, Arabic, Chinese as well as in English) a copy of the application form (in French, Spanish, Arabic, Chinese as well as in English), a list of WHP providers, a list of WHP Pharmacies, and a copy of the Poverty Guidelines.

Special note was made about the Emergency 90-Day Coverage. The were told that patients are enrolled only in 90-day coverage with full coverage contingent upon proof that they do not qualify for another program and/or submission of missing documentation. They were told that during this 90-day period, they must apply for Department of Human Services (DHS) benefits and have Medicaid or Adult Benefit Waiver (ABW) Program eligibility determined. They were also told that the current ABW program is frozen. Patient was provided with information on how to complete their (DHS) Medicaid application, a copy of the application and address and phone number of local DHS office. They were told that if they do not complete the DHS portion of the application they will not receive Emergency 90 Day Coverage.
