Community Outreach Program

The Community Health Nurse acts as a client advocate between client's physicians and other health agencies and provides client education when necessary. The nurse refers clients to other health services when necessary. Provides follow up care when necessary and documents all unmet health needs. Works collaboratively with other health care agencies to provide culturally appropriate service. Provides health care services to clients who are unable to travel to the health clinic for services.

Plans and organizes in-service for clients in cooperation with other health care agencies. Transports clients when necessary to health care appointments. Acts as an advocate for quality health care for clients as necessary and appropriate. Collects and organizes data related to services and the number of community members utilizing those services. Provides direct care to clients who are unable to travel to the clinic providing the services are within the scope of practice, care is given under the supervision of licensed health care providers.
