Contract Health

Contract Health Services (CHS) is a federally funded program run by the LTBB of Odawa Indians. CHS is not an entitlement program or an insurance plan. If a member does not have another resource, such as private insurance, Medicare, or Medicaid, that member will be required to apply for Medical Assistance to exhaust all resources. Ineligibility for medical assistance needs to be established to satisfy payer of last resort criteria. All forms necessary to apply of Medicaid are available at the LTBB Health Department. In one does not apply for medical assistance, the referral or request for payment will be denied. Indian Health Service does not guarantee that funds will always be available.

Eligibility

To be eligible for Contract Health Services (CHS) Tribal membership or proof of descendancy is required.

Utilization

LTBB health clinic must be accessed if appointments are available. Prior authorization for medical services and a reference number is required before service is given. When the clinic is not open, notification must be received the next business day. All prescriptions must be called into the Contract Health Department; the Contract Health Department will then notify the pharmacy that approval has been given. The Pharmacy will not distribute any prescriptions without approval. No over the counter medications will be authorized.
