 What can Social Workers do to assist the patient in Initiating Enrollment?

Social Work can assist by helping the patient gather needed information and fxing the information to the WHP at 734-544-6726.

The following documents should be completed to be faxed. You need to complete and use the WHP fax as well.

1. Completed WHP Application:

Please note the following questions and areas that are easy to miss and must be completed or enrollment will be delayed.

2. Identification

A copy of a Michigan Drivers license or Michigan ID card with a current Washtenaw County address must be submitted.

3. Income Information:

The patient will need to supply WHP with copies of income documentation (check stubs, tax forms, etc). A letter from the Social Worker (on letterhead) reporting patient’s stated income can be substituted to initiate emergency coverage. For patients who do not have an income, there is a zero-income statement available online for the patient to sign and date which acknowledges they have no income and promises they will follow-up with DHS as they may be eligible for ABW (as program is available) or Medicaid.

Items of Note:

Social Work can request that the patient’s “start date” for WHP eligibility be retroactive back to the 1 st day of the current month. This option must be specifically requested by Social Work to take effect. Contact WHP Case Manager for approval.

If a patient was covered under the 90-day option and has not yet received acceptance or denial from Medicaid, Social Work can request that the patient’s coverage be extended an extra 30 days. Social Work or patient must contact the WHP Case Manager for approval.

Social Work can request information verifying enrollment. When SW is faxing an application to assist with enrollment, please use the WHP fax cover sheet (available in Ambulatory Care Manual/Financing Medical Care/Washtenaw Health Plan). Social Work can request that a copy of the patient’s card be faxed to the Social Worker for our records and for the pharmacy. The patient will receive their card as well as enrollment materials in the mail.
